You can fill out the user-friendly forms in the Adobe Reader application.

If you cannot open these documents on your computer after downloading, you will need to install Adobe Reader by selecting the icon labeled, “Get Adobe Reader to view documents” at the bottom of the page.

After you complete filling out the 2 forms, please print and bring the hardcopies in to your initial appointment.
